Choose Google Search ads when the first job is to meet people who are actively searching for a relevant service. Choose Facebook and Instagram ads when the first job is to introduce a message or offer to people while they are browsing. Either channel can waste money when its job, creative, destination, and office follow-up are poorly defined, and neither channel guarantees appointments.
Give each channel one clear job
Google says Search ads can appear near search results when a person searches with terms related to an advertiser’s keywords. That makes the search itself an important intent signal, although it does not prove that every click is a suitable inquiry. See Google’s Search Network overview.
Facebook and Instagram advertising enters a feed and creative environment. The practice must earn attention with a useful idea, recognizable local context, or a clear offer before asking for the next action. This distinction is a planning tool, not a prediction. A high-intent search can still be irrelevant, and a feed ad can reach someone who later takes a meaningful step.
Use a five-question channel worksheet
- What must happen first? Capture existing demand, explain an unfamiliar service, fill a specific schedule gap, or introduce a new associate?
- What evidence of intent is available? A search query, a form answer, a connected call, or only a click or video view?
- What can the practice produce? Search campaigns need relevant ads and destination pages; social campaigns also need a dependable supply of credible images, video, and messages.
- What can the office handle? Name the responder, coverage hours, appointment capacity, and outcome-reporting owner.
- What result decides the next step? Define relevant inquiry, booked appointment, attendance, cost categories, and the review window before launch.
Compare the work behind the media
For Google Search, review the actual search terms, geographic settings, ad-to-page message, and conversion actions. Someone searching for a chiropractor may be researching jobs, education, insurance, or a service the office does not provide. The search-terms review is therefore part of weekly campaign management, not a one-time keyword exercise.
For Facebook and Instagram, plan who will record, approve, and refresh the creative. A strategy that depends on a doctor’s weekly videos is not workable if no recording time exists. Decide whether the ad sends people to a website, form, call, or another supported destination, then measure that stage separately from later appointment outcomes.

Sequence tests around the current constraint
Consider a hypothetical established practice adding appointment capacity for a service already described clearly on its website. Local search demand is visible, the office answers calls during business hours, but the practice has little recent video. A focused Google Search test may be easier to evaluate first because the intent, page, and response path are already available.
Now consider a practice introducing a new associate whose approach and schedule need explanation. The team can record useful short videos and handle inquiries, but few people know the associate yet. A Facebook and Instagram creative test may better match the first communication job. These scenarios illustrate sequencing; they do not predict which channel will produce a lower cost.
If both channels run together, give each a distinct hypothesis or use a measurement design that acknowledges overlap. Do not add each platform’s attributed conversions and call the sum unique patients.
Read results by stage
Cheap clicks can coexist with poor inquiry quality. Expensive clicks can coexist with relevant inquiries. Report spend, clicks or visits, connected contacts, relevant new inquiries, booked appointments, and attended appointments as separate rows. Allow the same cohort time to progress before comparing channels, and disclose management, creative, page, and software costs.
Change one meaningful variable at a time when practical. For Search, that might be query coverage or message match. For social, it might be the creative concept. Keep a dated log so a later result can be connected to the actual change without claiming causation from timing alone.
Choose the next test, not a permanent winner
The better first channel is the one that fits the practice’s current job and can be measured honestly. Revisit the decision when demand, capacity, creative resources, or the website changes.
